How does the NDIS ask for your information?

When the NDIS requests personal information from you, your representative, or your child's representative, it might be:
  • face-to-face, such as during your planning session
  • via phone call
  • through mail
  • via email or other digital means like video calls
  • on the Access Request Form you fill out when applying to join the NDIS as a participant
  • on the Supporting Evidence Form for providing evidence when asking for home and living supports
