Medical Errors can be devastating because you are doing a medical thing that can be detrimental to your health knowingly or unknowingly. So it is expedient to always be careful to avoid performing any medical errors that you might regret for a long time because of the adverse effects from the errors

So to prevent this from happrning you should be doing the below to be very sure of what you doing :

Try to show your Doctor or any medical personnel your drugs before consuming them

Do more of buying drugs that are mostly prescribed by your doctor

Ask about the side effects of a drug and discuss with your doctor if you think you react to any medicine

Always speak up with your doctor if you have any concern or questions about your health.

Always give your caregiver and doctors all the information about your health and medical history. You would be given a treatment based on the information you give.

Honor a test that a doctor want you to undertake, take the test and get the result for your doctor. This is for your own medical good.

Avoid storing drugs in large quantities at home. some might get expired without your knowledge, so that taking them become a problem for you.

You are right with your list it is always important to know those that are in charge of your health so that you know exactly the proople to meet for your health immediately you are down . Some hospital even allows for patients having their doctor numbers this will enable them to reach for medical advice whenever it is needed. Again, even getting close to a physician isn't a bad idea so that you can even have home care services when the needs arise.

Asking questions is one thing we need to always try to do rather than trying to shy away. I know soje doctors that would even asked one to ask questions whenever need be. I know too that some physicians can be very busy but it will still be good to find out issues from the physicians than just assuming. one's health doesn't need assumption. You need to precise to get better treatment.

The last one is that one should try to disclose all facts to the doctor. No need hiding any disease it will be to one detriment.
