Confusion regarding the non financial applications of Blockchain

1) Who validate or mine the blocks in non financial applications of Blockchain ?
(2) From whom/where the miner will get reward in non financial applications of Blockchain ?
(3) Considering Healthcare application, Who will write the details of medical report of Patient. If Physician, then how ? because Physician don't know the Private key of the patient.
(4) Can we track the Health record of ant patient or it is permission Blockchain

O really don't know who manages the data's on the blockchain network. I'm not familiar with those financial applications of the blockchain. All i know is that people that recognised it are there managing it. Everything in this life has its solution, that's why their are vast areas of expertise on earth. Everyone specializes on any given field to support in which way he can, to lift the society up.
